Who Can Make a Referral?
We accept referrals from NDIS participants, families, carers, support coordinators, allied health professionals, and other service providers.
What Happens Next?
Once we receive your referral, we’ll be in touch to discuss the participant’s needs, answer any questions, and outline next steps for assessment and support.
What We Need to Know
Please provide as much detail as you can in the form below. The more we understand about the participant’s situation, goals, and current supports, the better we can tailor our approach from day one.

Urgent Situations
If the participant is at immediate risk of harm to themselves or others, please contact Emergency Services or call 000. Our referral process is designed for non-emergency support planning. We are not a crisis service.
